Senior Health Services

We call our service the Polypharmacy Screen, Senior Health Services (SHS)*. Licensed healthcare associates visit the homes of local elderly at their request, to review their Medication usage and overall health concerns, more-recently including their 'Adjunct Individualized Diet/Exercise' (AIDE) needs. The key goal is to keep the elderly out of the hospital where they are at risk for hospital-based infections, which are sometimes terminal, but also susceptible to one of the 2.7 million medication errors that occur each year in American hospitals. Such errors are extremely costly to the healthcare system, resulting in additional and unnecessary expenditures of as much as $177 billion per year. The Senior Health Services (SHS)* program evolved from a small pro-bono solo medical specialty practice established in 2005 by Dr. Leon Lewandowski in Westhampton, NY. Dr.Lewandowski had spent much of his earlier medical career in the pharmaceutical industry developing drugs and drug-delivery systems, with special emphasis on safer and more efficient medication-usage, especially by the elderly. Over time, SHS has expanded into a larger program utilizing professionally-licensed, community-based nursing personnel local to Long Island, whom we train to provide the national standard of home care services outlined under "Services". Our New York State-recognized 'not-for-profit' program (officially designated the 'Polypharmacy Screen, Inc.') remains currently pro-bono, until our services become formally-recognized as billable to government and private insurers either on a "fee-for-service" under a Federal Wellness Program or "capitation-basis", with little or no out-of-pocket cost to the patient.
